Adult Day Health Care
The mission of the Adult Day Health Care Program is to provide those who are entrusted to our care with a supportive, nurturing and family-centered environment that promotes wellness, independence and self-esteem, and that recognizes and celebrates the value of the individual.
Is this program a medical or social model?
Our Adult Day Health Care Program is a medical model program, operated under the supervision of a registered nurse. Our program provides comprehensive healthcare with therapeutic social, educational and recreational activities. Our program enhances the lives of the frail elderly, the chronically ill and disabled adults while maintaining their residence in the community. At the same time, we can provide a period of respite for family caregivers.
What services does Adult Day Health Care Program provide?
Registrants are offered a wide range of services under the direct supervision of a registered nurse and the registrant’s physician. These services are provided or arranged during program hours:
- Nursing care
- Nutritious meals
- Physical, occupational and speech therapy
- Aqua therapy
- Case management
- Social work
- Psychological counseling
- Therapeutic activities
- Dental assessments
- Health education
- Pastoral care
- Laboratory services
- Barber/beautician services.
What activities does the Adult Day Health Care Program offer registrants?
We provide a busy, active day for our registrants. Recreational and social activities include:
- Shopping trips
- Religious services
- Pet therapy
- Holiday events.
We also offer group exercise, walk groups and individualized exercise programs. Registrants also participate in community events and fundraisers.
What about transportation?
We provide door-to-door transportation for Adult Day Health Care registrants within a ten-mile radius of the program. The bus accommodate wheelchairs and assistive devices.
When is the program open?
Our hours are Monday through Friday from 8am to 4pm.
Who is on the Adult Day Health Care team?
The St. Luke’s Home Adult Day Health Care Program team is composed of:
- Program director
- Registered nurse
- Social worker
- Physical, occupational and speech therapists
- Certified nursing assistants
- Support staff.
