Becoming A Dental Patient

Prior to acceptance we require prospective patients to fill out a patient information sheet and medical history form followed by a 'screening appointment.' Screening does not guarantee acceptance. Many factors influence your acceptance into our educational program including but not limited to: the complexity of your dental health and your Medicaid eligibility.

What Will Happen At The Screening Appointment?

Your screening appointment will consist of the following:

  1. The Program Director will review your completed medical and dental history forms.
  2. Two tongue blades will be used for a brief glimpse of your oral condition. 
  3. A panoramic radiograph (x-ray) may be ordered to review your current dental status. 
  4. If accepted for treatment, you will be scheduled for a comprehensive examination and hygiene appointment.  


Patients who do not qualify to participate in our clinical educational program will advised at the screening appointment. We regret that all patients screened cannot be accepted for dental treatment. Your treatment may be too complex for our resident dentists and may be best managed by a private dentist.

What Kind Treatment Is Provided?

If you are a Medicaid recipient, please be aware that not all procedures are covered by Medicaid. Once accepted as a new patient, you should discuss all planned treatment with your assigned dentist so that Medicaid coverage can be determined before treatment begins.

Emergency Dental Care

Patient 's who are in need of emergency dental treatment because of severe ongoing pain, infection or swelling should contact our office for specific scheduling information. Screening appointments are not required for emergency dental treatment. Since emergency patients are generally worked in between our regular scheduled patients, longer waiting times can be expected.

How Long Are Scheduled Appointments At The Dental Health Center?

Since we are a teaching facility, the length of your appointments and overall treatment will likely take longer than it would from a private dentist. High standards are required of our resident dentists, and their work is continually being evaluated by our clinical faculty. This attention to detail requires more of your time, but assures you of quality dental care. If your schedule is such that it is difficult for you to come frequently and stay for the more lengthy appointments often required, then you should consider seeking care from a private dentist.

Who Can I Contact?

Contact the Faxton-St. Luke's Healthcare Dental Health Center receptionist at (315) 624-6227 between the hours of 9:00 A.M. and 11:30 A.M. or 1:30 P.M. and 4:00 P.M. to request a screening appointment.

What Kind Of Dental Coverage Is Accepted?

Faxton-St. Luke's Healthcare Dental Health Center accepts patients with traditional Medicaid coverage only.  At this time we do not participate with Fidelis or CHP (Child Health Plus) Insurance Programs.

Patient Treatment Hours

Patient care hours are BY APPOINTMENT ONLY from 8:45 AM to 12:30 PM and 1:15 to 5:00 PM, Monday through Friday. The Center is closed weekends and holidays that are recognized by Faxton St. Luke's Healthcare.

About Relationship-Based Care

What is Relationship-Based Care (RBC)?

With RBC, nursing is just one part of the healthcare delivery picture. It takes an entire organization to care for you. Nursing plays a significant role in your care and recovery, but each and every employee has a piece in creating a positive experience for you.

Relationship-Based Care (RBC) was adopted by FSLH following the consolidation of Faxton Hospital and St. Luke’s-Memorial Hospital in the fall of 2005. FSLH partnered with Creative Health Care Management to build and sustain this model of patient care which has helped FSLH become a healthcare organization that provides a positive, caring and healing experience for you and your family. It has been instrumental in guiding our employees and creating exceptional teamwork.

What are the principles of RBC?

  1. Responsibility for relationship and decision making - a registered nurse manages your care from admission to discharge
  2. Continuity of care through work allocation and patient assignment
  3. Communication
  4. Management
  5. Process improvements
  6. Providing a caring and healing environment of care

Every department is represented at the table and has a voice in making decisions about how care and service are provided in their own area. Employees in our 60 units and departments continually look at processes and procedures and ask, “How can we do this even better and make sure that the patient and their family are always our central focus?”

How do employees have a voice?

Each of FSLH’s 60 departments has a Unit Practice Council (UPC). The UPC is a small group, usually 8-10 people, who meet regularly to discuss issues/challenges that have arisen and work on solutions to improve them. Unit Practice Councils are comprised of individuals who are selected by their peers and accept the responsibility, authority and accountability to represent the staff on the unit. UPC members focus on implementation of the RBC principles, always putting the you and your family at the center of all they do.

Once a month, all 60 UPCs send a representative to a meeting where the details of creating the best possible patient experiences are discussed and coordinated among all the members.

Working together, UPCs have helped to streamline patient transport, address dietary issues and expedite linen delivery, to name a few. They have also improved employee satisfaction, which we believe is integral to delivering quality healthcare – happy employees lead to happy patients.

Are other healthcare organizations doing this?

Organizations that preceded Faxton St. Luke’s Healthcare with implementation of the Relationship-Based Care model only involved nursing. FSLH was the first in the nation to roll out RBC system-wide.

In July, 2009, Faxton St. Luke’s Healthcare was on the national stage as our organization took part in the National Relationship-Based Care Symposium held at the Turning Stone Resort. The symposium, hosted by Creative Health Care Management, showcased our employees and the great strides FSLH has made on our RBC journey. Leaders in diverse roles at all levels of healthcare organizations from across the United States attended and several toured FSLH to speak with staff and further examine our success.

Since the symposium, healthcare organizations across the country have contacted FSLH to learn how we were able to engage the entire organization, every employee in every department. As part of the Keith A. Fenstemacher Center for Continuous Learning, we have begun a formal two-day program for organizations to attend to learn first-hand about our road to success.

Our journey isn’t over; we are still working to further improve the patient and family experience. It isn’t always easy, but it’s what we are passionate about. The commitment of our leadership and the hard work and dedication of our more than 3,000 employees have made this exciting transformation possible. As we continue our journey, we are reminded about the two things that make Faxton-St. Luke’s Healthcare special – It’s the people. It’s the care.

SUNY Canton has partnered with Mohawk Valley Community College (MVCC) Health Services Department to offer this jointly registered program in Rome, New York.

General Education Courses

The general education courses can be taken online through SUNY Canton's course management system or at MVCC's Rome Campus on Floyd Avenue.

Dental Hygiene Theory Courses

The dental hygiene theory courses will be offered in a hybrid delivery. SUNY Canton's highly qualified faculty combines traditional classroom instruction with on-line instruction, meaning students have unlimited access to course lectures and interactive learning activities.

Clinical and Laboratory Courses

The clinical and lab courses are taught at the Donald J. Mitchell Rome Veterans Affairs Outpatient Clinic building located at 125 Brookley Road, Rome, New York. Depending upon the semester, students spend 8-15 hours a week in the clinical setting learning, practicing and
mastering the functions written in the NYS Dental Practice Act. The students progress from practice on typodonts and fellow students to providing oral health services to patients from within the community. A dentist and several registered dental hygienists who hold advanced degrees closely supervise the students as they interact with the patients in the clinic. A low faculty/student ratio, in both clinic and labs, allows for individualized instruction, immediate feedback and continual growth.

Faxton-St. Luke's Healthcare Dental Health Center and St. Luke's Home will provide additional clinical experience for students in the second year of the program.

Dr. Michelle Haddad Memorial Seminar

Dr. Michelle Haddad was born on October 15, 1959 to Dr. Louis and Gloria Haddad and attended local schools. She graduated from Utica College in 1981. She attended the S.U.N.Y. at Buffalo School of Dentistry beginning September of 1981. She served a summer Externship at St. Luke's-Memorial Hospital Center during the summer of 1984. Dr. Haddad graduated from dental school in May 1985 followed by the General Practice Residency Training Program in Dentistry at St. Luke's-Memorial Hospital Center from July 1, 1985 to June 30, 1986.
