I am so happy you are considering volunteering at Faxton St. Luke’s Healthcare. Not only will you be helping other people, you will have that self-gratification it brings when you extend your time for others.
Another incentive is if you volunteer 4 hours or more on a given day you will be entitled to a complimentary meal.
A few things we need from you before you start…
- Completed volunteer application with 2 references then…
- Health records, including a copy of a physical dated within the last 12 months and shot records to show that you have had 2 MMR’s
- Proof of a Mantoux /PPD (Tuberculosis Skin Test) – taken within 3 months. You can contact the Employee Health Office at 624 – 6391to schedule a test
- Completed orientation sign off and post test
- Volunteer Handbook sign off
- Code of Conduct sign off.
Once you have completed all of the paperwork, you can drop it off at Faxton St. Luke’s Healthcare’s Volunteer Office at the St. Luke’s Campus or mail it to:
