Qualifications:

Dietetic Technician

Description: 

The Dietetic Technician serves as a nutrition representative on assigned units and works with the Nutrition Services team to optimize nutritional care/education of the patient’s and their families.  The primary responsibilities include providing and assuring the highest quality of nutritional care to customers at all times.  Direct patient care includes assessing and monitoring the nutritional care of patients and assessing their understanding and compliance of diet education.  The Dietetic Technician is able to serve clients of all age groups.  The Dietetic Technician works proactively with all related departments to optimize nutritional care and to recommend medical nutritional therapy as needed.  Assigns risk levels, conducts low to moderate risk nutrition assessments and prepares nutritional care plans for patients.  The nutrition care plan considers age specific needs, physical or cognitive limitations/barriers, as well as cultural, religious and ethnic concerns.  The Dietetic Technician helps to oversee the diet clerks on a daily basis, as well as help to oversee trayline as the trayline checker and pursues personal development through continuing education programs.

Qualifications: 

Associates Degree from a program which meets the educational needs by the American Dietetic Association.  Registration by the Commission on Dietetic Registration preferred.  Evidence of continual update of education appropriate to the position as a result of own initiative.

Qualifications:

Qualifications:

Patient Flow Coordinator*

Responsible for 24/7 management of 10 bed observation unit including staffing and fiscal management. Qualifications:

RN Quality Care Consulatant*

The Quality Care Consultant RN concurrently assesses, plans, implements and evaluates the needs of patients for care coordination, discharge planning, utilization review and quality review.  Utilizes Utilization Review procedures including those required for discharge planning, in addition of knowledge critical for Medicare, Medicaid coverage, HMO, or private insurers to evaluate the quality of care and need for hospitalization.  The Quality Care Consultant assumes a leadership role in the coordination of discharge planning with the patient/family, healthcare providers, referral agencies and other facilities.  Monitors and concurrently assesses the quality of care delivered by physicians, nurses and ancillary support services staff using specific approved criteria.  Performs concurrent review of medical records to ensure appropriate clinical documentation, quality indicator data (i.e. Core Measures) and other information is present and utilizes computerized tools for data collection.  Functions as a member of the care team supporting patient safety and performance improvement activities throughout the organization.

Qualifications:
