Adult Day Health Care Program Expanding and Accepting New Registrants

Faxton St. Luke’s Healthcare’s (FSLH) Adult Day Health Care Program is a community-based, long-term care program that provides comprehensive health care with therapeutic, social, educational and recreational activities. The program enhances the lives of frail, elderly, chronically ill and disabled adults while maintaining their residence in the community. The program also offers a period of respite for family caregivers.
In early 2013, FSLH’s Adult Day Health Care Program will relocate to the new, state of the art Center for Rehabilitation and Continuing Care Services (also the location of St. Luke’s Home), 1650 Champlin Avenue, Utica. The larger location supports the Adult Day Health Care Program’s mission to provide registrants with a supportive, nurturing and family-centered environment that promotes wellness, independence and self-esteem.
Adult Day Health Care offers a wide range of services including nursing care, nutritious meals, social work, psychological counseling, health education, therapeutic activities and therapy. The program also provides door-to-door transportation services for their registrants within a 10 mile radius. The Adult Day Health Care Program is open 8am to 4pm, Monday through Friday. The team includes a program director, registered nurse, social worker, psychologist, physical, occupational and speech therapists, dietitians, pharmacist, certified nursing assistants and support staff.
