I need my medical records. How can I access them?
The Health Information Management Department (HIM) can assist you with accessing your individual patient medical record.
To view your medical records
You may view your medical records during regular business hours, Monday through Friday, 8:00am to 4:00pm, by making an appointment with the HIM department at the St. Luke’s Campus, 1656 Champlin Avenue, Utica, New York.
How do I get copies of my medical records?
You may receive copies of your records within ten business days after providing us with a written request (see attached).
Is there a fee for copies of my records?
A fee of $0.75 per page is applied for requests in excess of ten pages. If your request for records is for continued medical services, the fee does not apply.
Are all of my medical records kept at the St. Luke’s Campus?
Some of your records, such as x-rays, may be stored in other areas of the facility. Below is a contact guide for the types of records you may request.
