The Refer a Friend AwardperQs are given to any current Faxton St. Luke's Healthcare employee who refers an outside candidate who is hired to fill a vacancy in one of the designated positions listed below. Refer a Friend AwardperQs apply to referrals for anyone who is hired into a position that works a minimum of five days (37.5 hours) per pay period.

How to Qualify for the Refer a Friend Porgram
The candidate must indicate your name on the employment application when applying for one of the above-designated positions.
When do I receive my $500 and AwardperQs?
- The $500 will be paid out after 90 days
- 50 AwardperQs will be deposited into your personal Applause account after 90 days
- 150 AwardperQs will be deposited after 6 months
Rules for Qualification
- You must be a current employee and maintain employment throughout the period to be eligible.
- The Refer a Friend AwardperQs will not be paid if the hire previously workd for FSLH in one of the above eligible positions and was terminated less than one year from date of rehire.
- Refer a Friend AwardperQs previously paid cannot be paid again on the same individual for the same position.
NOTE
Excluded from participation in the program are management and supervisory staff, Human Resources Department staff, and employees who are adjunct faculty with affiliated schools/colleges.
**Positions will be updated quarterly based on market fluctuations and will be at the discretion of the Human Resources Department.
